AI Incident Response

AI incident response is the set of processes used to detect, contain, investigate, and learn from harmful model behavior in the real world. This might include hallucinated advice, leaked data, toxic outputs, or an agent taking the wrong action.

A Typical Response Flow

  1. Detect the issue through alerts, audits, or user reports.
  2. Contain the damage by disabling a feature, model, or tool pathway.
  3. Investigate the prompt, context, model version, and logs.
  4. Patch the issue and add a regression test.
  5. Document the root cause and the mitigation.

Why AI Incidents Are Different

The failure may come from data, prompts, retrieval, tool use, or the model itself. That means incident response needs both software debugging and behavioral analysis.

Mature Teams

Strong teams plan for incidents before they happen. They log enough information to reconstruct failures and they define who can pause or roll back AI features quickly.
